    Request for a Program Plan

    Please fill out this form to request a program plan. Although every effort will be made to ensure the accuracy of the program plan, ultimately it is the students responsibility to confirm that they are eligible to complete the courses they are registered for and that they are meeting departmental and university requirements. Students must read course descriptions carefully to ensure that they have the pre-requisites for courses (or permission) and that courses they are registered in will not replaces grades and credits in courses they have already completed. Advisors will not assess for these issues when preparing the program plan. 

    Please note that students should be especially mindful of any recent changes they have made to their registration, our database does not update continually so advisors might not be aware of any recent changes. 

    Please choose the stream you intend to complete, even if it is different from the stream you are officially registered in.
